Medical Forms and Information
Pre-Participation Exam (PPE)
Before competing in athletics at Saint Mary’s College all new/transferring student-athletes must complete a pre-participation exam. This will be scheduled and conducted IN PERSON by the athletic training staff.
Student-athletes must attend an AT-Check-in before they can be scheduled with Health & Counseling for a physical. This ensures that all student-athletes have their paperwork filled out correctly, concussion baseline testing completed, and musculoskeletal exam finished. This helps streamline the physical examination process.
This will be completed BEFORE a physical is scheduled with SMC’s team physician or Nurse Practitioners. All medical paperwork must be submitted and approved before an appointment is scheduled through Health & Counseling for a physical. All physicals must be completed on campus prior to participation.
PPE Steps:
-
Create Healthy Roster account
-
Complete medical paperwork assigned to your account
-
Check back to make sure it was all approved or if corrections are needed
-
Attend an AT-Check-In with one of the Athletic Trainers
-
Physical appointment with Health & Counseling
Sickle Cell Trait Results/Testing
All new/transferring student-athletes are required to provide documentation of their sickle cell trait status. It is recommended by the NCAA and required by Saint Mary’s College to provide this documentation prior to competition. If documentation is not provided before arriving on campus the test will be completed during their physical in the Health and Counseling Center. This should come as no charge to you as it is run through both primary and secondary insurances.
Pre-Participation Medical Disclosure Requirements
If you have been diagnosed with any cardiac or respiratory conditions, diabetes, or have undergone any recent surgeries, these must be disclosed in your pre-participation medical paperwork. This disclosure allows our medical staff to monitor your condition appropriately and to develop a care plan in case of an emergency.
For recent diagnoses, we ask that you provide documentation from your diagnosing physician. This should include recent test results, your current management plan, and any other relevant information. If the Saint Mary’s College team physician determines that additional testing is necessary before you are cleared for competition, you will be informed during your scheduled physical examination.
Prescription Medication Disclosure
If you have been prescribed medication for any medical condition, you are required—by both the NCAA and Saint Mary’s College—to provide the following information along with your pre-participation paperwork:
This documentation must be submitted before your scheduled physical examination.
Please note that some medications, including certain ADHD treatments, contain substances banned by the NCAA. If you are selected for drug testing, having a prescription already on file enables us to submit a medical exemption application to the NCAA on your behalf. This is essential to prevent any potential sanctions related to your participation eligibility.
Healthy Roster
The Saint Mary’s College Athletic Training Staff uses Healthy Roster to store and maintain medical records for Belles student-athletes. Student-athletes are responsible for updating their personal information as well as having the necessary medical paperwork completed and filled out before participating in any official team activities. These activities include practice, team lifts, games, and nontraditional seasons. If you have any questions please contact Danielle Scheel at dscheel@saintmarys.edu.
The forms and documents are for all student-athletes participating in NCAA sports while at Saint Mary's. No individual will be allowed to compete, practice or participate in any form until these forms are completed and received by the athletic training staff.
